Your prescriptions are earning 5% back. Are you actually using it?

If you have a Prime Visa and an Amazon Prime membership, Amazon Pharmacy may be one of the more overlooked 5% categories in your wallet.

Amazon Pharmacy
Amazon Pharmacy is highly rated for fast, free shipping and convenience, often surpassing competitors like Express Scripts and Costco in user experience. It excels with Prime member discounts (up to 80% off generics) and $5/month RxPass for select generics, offering transparent, competitive, and often lower costs compared to traditional retail pharmacies.

Here's a reminder that’s easy to miss: every time you fill a prescription through Amazon Pharmacy and pay with your Prime Visa, you can earn 5% back. It’s not a promotion or a limited-time offer—it’s part of how the card works, though many cardholders don’t realize it applies to pharmacy orders.

The short version: Amazon Pharmacy is generally processed as an Amazon.com purchase. The Prime Visa earns 5% back on Amazon.com with an eligible Prime membership. That typically includes Amazon Pharmacy and PillPack orders.

Why this matters more than you think

Most rewards cards earn around 1% on pharmacy purchases—sometimes up to 3% with specific categories. By comparison, the Prime Visa’s 5% on Amazon Pharmacy can function like an ongoing pharmacy bonus for eligible purchases, alongside any built-in Prime medication discounts.

This can apply to many common prescriptions—generic and brand-name—when ordered through Amazon Pharmacy and paid for with your Prime Visa. Rewards are typically credited shortly after the transaction and can be used toward future Amazon purchases.

That said, availability may vary depending on the medication, insurance plan, and state regulations, so not every prescription or situation will qualify.

How to actually set this up

1
Confirm your Prime Visa earns 5%

Go to Your Payments in your Amazon account and expand your card details. You’ll see whether you're earning 3% (non-Prime) or 5% (with an eligible Prime membership). If it shows 3%, check that your Prime membership is active and linked to the same account.

2
Set up or transfer to Amazon Pharmacy

Visit pharmacy.amazon.com. You can request a prescription transfer from another pharmacy, or have your doctor send a new prescription directly.

3
Select your Prime Visa at checkout

When placing your order, manually choose your Prime Visa. If you have multiple cards saved, double-check that the correct one is selected.

4
Compare prices before paying

Amazon Pharmacy often shows both your insurance copay and a Prime cash price. In some cases, the Prime price may be lower than your insurance copay—making it worth paying out of pocket and earning 5% back. Results will vary by medication and plan.

A few things worth knowing

Important caveats

  • If you have multiple Prime Visa cards on one account, only the primary card may receive the full 5% rate; others may earn 3%.

  • FSA/HSA-eligible purchases can qualify for 5% back if the Prime Visa is used as the payment method. (Many people instead use a separate benefits card, which would not earn rewards.)

  • The 5% applies only to what you pay out of pocket—not the total cost of the medication. For example, a $10 copay earns 5% on $10.

  • Not all medications are eligible through Amazon Pharmacy (for example, certain controlled substances), and service availability can vary by location.

Also worth mentioning:
If you take multiple ongoing generic medications, Amazon’s RxPass allows eligible Prime members to pay a flat monthly fee (currently $5) for access to a set of covered generics. In cases where you regularly fill several eligible prescriptions, RxPass may offer more predictable savings than pay-per-order cashback. For occasional prescriptions, using your Prime Visa for 5% back may still be the simpler option.

If you already have a Prime Visa, an active Prime membership, and out-of-pocket prescription costs, it may be worth checking whether Amazon Pharmacy fits your needs. Setting it up can take a few minutes, and the 5% back applies automatically once everything is in place.

It’s a straightforward benefit—but one that’s easy to overlook if you’re not thinking of prescriptions as part of your rewards strategy.
